Appeal a Social Security Decision Application Form


You may appeal against the Social Security Director’s decision before the Social Security Umpire. Appeals may be lodged only on any question of law or a principle of importance  emerging from the Director’s decision according to the Social Security Act. The appeal has to be lodged within 30 days of the day you have been i​nformed of the decision.

Injury Benefit Application Form
 
  
A person, who suffers personal injury caused by accident arising out of or in the course of his/her employment or self-occupation, or has developed a disease, may be entitled to Injury Benefit/Industrial Disease. A person may apply for this benefit using this form. Once the form is submitted to the department, the application is passed on to a Medical Panel for professional review. Following the review, the applicant will be contacted and given the appropriate instructions.

Request for Entitlement to Remain Insured in Malta (Form A1)

 

The A1 form certifies which social security legislation applies to the holder of the form. It will generally be needed in situations where you, as an employed or self-employed person, have a connection through your employment or self-employment with more than one EU country. Under the EU rules a person is only subject to the legislation of one country at any one time. The A1 is issued by the country to whose legislation you are subject and confirms that you are not subject to the legislation of any other country you are connected with – in other words you will have no obligation to pay social security (including health insurance) contributions in the other countries. The A1 remains valid until the date of expiry, indicated at the form or until it is withdrawn by the issuing institution.

Retirement Pension Application Form

 
You can make use of this form if you have received a notification letter from the Department of Social Security requesting you to provide the details necessary to start off your retirement pension assessment. This application form may also be used to apply for a Retirement Pension.
 

Retrospective Payment of Social Security Contributions

 
Eligible applicants can pay back-dated Social Security Contributions which are missing from their respective contribution history, in such a way that they can either qualify for a contributory pension, or else enhance its current or potential payment rate.
 
 

Senior Citizen Grant

 
The Senior Citizen Grant is a onetime payment but paid every consecutive year. Payment is made to those elderly persons who have already reached their 75th birthday or on a pro-rata basis from the date of the 75th birthday up to the end of the current year.
 


Social Assistance and Unemployment Assistance Application Form

 
This benefit applies to heads of households that are incapable of working due to medical reasons or are unemployed and seeking employment and whose financial means fall below the treshold estalished by the Department of Social Security.

Social Security Number Application Form

 
Historically, your social security registration number is a number listed under the Social Security Act (Cap. 318) through which you pay social security contributions. The social security contributions paid will entitle you for the social security benefits provided under the Social Security Act (Cap. 318), subject to the entitlement conditions which exist for each type of benefit. You can find more information on the categories and amount of social security contributions employees and self-employed/occupied persons need to pay here.

Persons in possession of a permanent Maltese I.D. card will be automatically registered under the social security act once they have reached 16 years of age. Persons not in possession of a permanent Maltese I.D. card between the age of 16 and pension age need to apply for a social security registration number. This number is only issued once and is valid throughout the lifetime of the applicant.
