Appeal against a Decision by the Department of Social Security Application

 
You may appeal against a decision by the Department of Social Security before the Social Security Umpire. Appeals may be lodged only on any question of law or a principle of importance emerging from a decision by the Department of Social Security according to the Social Security Act (Cap. 318.).

Marriage Grant Application

A person who has recently married or entered in a civil union will be eligible to apply for a Marriage Grant once the criteria stipulated in the Social Security Act (Cap. 318.) are fulfilled.
 
You can make use of this application if you have received a notification letter from the Department of Social Security requesting you to provide details necessary to start off your Marriage Grant assessment. This application may also be used to apply for a Marriage Grant.

Maternity and Adoption Benefits Application

 

A pregnant woman or a person who adopts a child and fulfills the criteria set out by the Social Security Act (Cap. 318.) may apply for these benefits.

Maternity Leave Trust Claim Application
 

This application is intended to be used by employers that would like to claim a refund on paid Maternity Leave that has been used by an employee within their employ. Employers that have successfully submitted this form will receive a refund once payments are processed by the Department of Social Security.

Milk Grant Application

​This benefit is awarded to those mothers who have the care and custody of a child less than forty weeks of age who requires weaning or complementary feeding or who cannot breast feed for medical reasons​.

Registration for a Social Security Number Application

 
Persons in possession of a permanent Maltese Identity Card will be automatically registered under the Social Security Act (Cap. 318.) once they have reached 16 years of age. Persons not in possession of a permanent Maltese Identity Card between the age of 16 and pension age need to apply for a Social Security Number. This number is only issued once and is valid throughout the lifetime of the applicant.

Request for Certification of Insurance/Employment periods in another EU Country (Form U1) Application

 
If you become unemployed after working in the country you have moved to and claim unemployment benefit there, the record shown on the U1 may need to be used to establish your entitlement to unemployment benefit. The same applies if you claim unemployment benefit in your country of residence after working in another country. When you claim unemployment benefit in such circumstances, therefore, you may have to present your U1 form to the unemployment service when you make your claim. If you do not present the U1, the institutions will exchange necessary information electronically.

Request for Entitlement to Remain Insured in Malta (Form A1) Application

 
The A1 form certifies which social security legislation applies to the holder of the form. It will generally be needed in situations where you, as an employed or self-employed person, have a connection through your employment or self-employment with more than one EU country. Under the EU rules a person is only subject to the legislation of one country at any one time. The A1 is issued by the country to whose legislation you are subject and confirms that you are not subject to the legislation of any other country you are connected with – in other words you will have no obligation to pay social security (including health insurance) contributions in the other countries. The A1 remains valid until the date of expiry, indicated at the form or until it is withdrawn by the issuing institution.

Retirement Pension Application

   

A person attaining retirement age may be entitled to a Contributory Retirement Pension, if s/he satisfies a set of Statutory Conditions.

You can make use of this form if you have received a notification letter from the Department of Social Security requesting you to provide the details necessary to start off your Retirement Pension assessment. This application form may also be used to apply for a Retirement Pension.

Retrospective Payment of Social Security Contributions Application

 
Eligible applicants can pay back-dated Social Security Contributions which are missing from their respective contribution history, in such a way that they can either qualify for a contributory pension, or else enhance its current or potential payment rate.

Senior Citizen Grant Application

 
The Senior Citizen Grant is a onetime payment but paid every consecutive year. Payment is made to those elderly persons who have already reached their 75th birthday or on a pro-rata basis from the date of the 75th birthday up to the end of the current year.

A person becomes entitled to Sickness Assistance if he/she proves to the satisfaction of the Director of Social Security that he/she or any member of his/her household is suffering from a disease that could only be cured or alleviated by a special diet or regimen, which incurs an exceptional expenditure.

Social Assistance and Unemployment Assistance Application

 
If you are a Head of Household who may be incapable of working due to medical reasons, unemployed and seeking employment, else registering under Part 1 of the Unemployment Register, or you are following a drug or alcohol rehabilitation therapeutic programme you may apply for Social Assistance or Unemployment Assistance.

When children turn the age of sixteen and are still attending school and without receiving a stipend, or registering for work but never worked before, are entitled to Student Allowance. Parents can qualify only if their income does not exceed the €24,833.

Social Assistance beneficiaries who find employment can opt for the Tapering of Benefit scheme if they earn at least the National Minimum Wage or more and if they have been in receipt of Social Assistance or Unemployment Assistance or Special Unemployment Benefit for at least two years in the last three years.

Tapering of Benefits Application (Head of Household - Spouse's Employment)

Social Assistance beneficiaries can opt for the Tapering of Benefit scheme in the eventuality of the spouse participating in a gainful occupation, earns at least the National Minimum Wage or more and if the beneficiary has been in receipt of Social Assistance for at least two years in the last three years.

Tapering of Benefits Application (Married/Civil Union/Cohabiting as per provisions of the Cohabitation Act 2017)


Beneficiaries in receipt of Social Assistance or Unemployment Assistance or Single Unmarried Parent Assistance or Special Unemployment Benefit for a period of two years in the last three years and contract a marriage or civil union or a cohabitation (Cap. 571.) with an employed person whose earnings are equal to the weekly National Minimum Wage or more.

Tapering of Benefits Application (Self-Occupied Person)
 
Social Assistance beneficiaries who start participating in a gainful occupation can opt for the Tapering of Benefit scheme if they earn at least the National Minimum Wage or more and if they have been in receipt of Social Assistance or Unemployment Assistance for at least two years in the last three years.

Tapering of Benefits Application (Single Parent)

Single parents with children under 23 years of age, are awarded Tapering of Benefits as long as they work at least ten hours per week at the National Minimum Wage rate per hour. In such cases, the requisite to have received social assistance for two years in the last three years does not apply.

Widow/er's Pension Application

 

A widow/er, whose spouse satisfies the relevant contribution conditions on his/her death, is entitled to a Widow/er’s Pension or to a Survivor’s Pension, whichever is the most beneficial.

You can make use of this application if you have received a notification letter from the Department of Social Security requesting you to provide details necessary to start off your Widow/er's Pension assessment. This application may also be used to apply for a Widow/er's Pension.
