Appeal against a decision by the Department of Social Security Application

 
You may appeal against a decision by the Department of Social Security before the Social Security Umpire. Appeals may be lodged only on any question of law or a principle of importance emerging from a decision by the Department of Social Security according to the Social Security Act (Cap. 318.). 
 
 

Children's Allowance Application - Parents Annual Income less than €24,833

 

The Children’s Allowance Benefit is given to parents having the care and custody of their children. The payment rate for applicants having an income less than €24,833 is determined according to their income.

Children's Allowance Applicaiton - Parents Annual Income exceeding €24,833

The Children’s Allowance Benefit is given to parents having the care and custody of their children. Applicants having an income greater than €24,833 will be paid a predetermined flat rate.

 

Children's Allowance Application - Additional Children


The Children’s Allowance Benefit is given to parents having the care and custody of their children.  Applicants having an income greater than €24,833 will be paid a predetermined flat rate. On the other hand, the payment rate for applicants having an income less than €24,833 is determined according to their income.



 

Children's Allowance Application - Claim for Revision 


If applicants experience any change in circumstances such as death, separation, marriage/co-habitation, change in custody or termination/start of employment they are to notify the Department of Social Security within a 6 month period.  

 

Child in Care Benefit Application - Foster Care Service 

 
Any family, married couple, and single individuals (in particular cases only), irrespective of their age, class, race and religion, can become foster carers after undergoing training, followed by an assessment, and approval by the Fostering and Adoption Panel.

Contributory Retirement Grant for Non-Pensioners Application

 
The Deficiency Contribution Bonus is applicable to persons insured in Malta for Social Security purposes, who are aged between 62 and 74 and who do not have enough Social Security Contributions to qualify for a Contributory Pension.

Injury Benefit Application
   
A person, who suffers personal injury caused by accident arising out of or in the course of his/her employment or self-occupation, or has developed a disease, may be entitled to Injury/Industrial Disease Benefit. 
 
 
 

Invalidity Pension Application 

   
A person who is certified as being incapable for suitable full-time or regular part-time employment due to a serious disease or bodily or mental impairment will be entitled to an Invalidity Pension, subject to the relative contribution conditions.

Marriage Grant Application

A person who has recently married or entered in a civil union will be eligible to apply for a Marriage Grant once the criteria stipulated in the Social Security Act (Cap. 318.) are fulfilled.


You can make use of this application if you have received a notification letter from the Department of Social Security requesting you to provide details necessary to start off your Marriage Grant assessment. This application may also be used to apply for a Marriage Grant.
  
 
 

Maternity and Adoption Benefits Application


A pregnant woman or a person who adopts a child and fulfills the criteria set out by the Social Security Act (Cap. 318.) may apply for these benefits.

Maternity Leave Trust Claim Application

This application is intended to be used by employers that would like to claim a refund on paid Maternity Leave that has been used by an employee within their employ. Employers that have successfully submitted this form will receive a refund once payments are processed by the Department of Social Security.

Request for Certification of Insurance/Employment periods in another EU Country (Form U1) Application

 
If you become unemployed after working in the country you have moved to and claim unemployment benefit there, the record shown on the U1 may need to be used to establish your entitlement to unemployment benefit. The same applies if you claim unemployment benefit in your country of residence after working in another country. When you claim unemployment benefit in such circumstances, therefore, you may have to present your U1 form to the unemployment service when you make your claim. If you do not present the U1, the institutions will exchange necessary information electronically.
  
 
 

Request for Entitlement to Remain Insured in Malta (Form A1) Application


The A1 form certifies which social security legislation applies to the holder of the form. It will generally be needed in situations where you, as an employed or self-employed person, have a connection through your employment or self-employment with more than one EU country. Under the EU rules a person is only subject to the legislation of one country at any one time. The A1 is issued by the country to whose legislation you are subject and confirms that you are not subject to the legislation of any other country you are connected with – in other words you will have no obligation to pay social security (including health insurance) contributions in the other countries. The A1 remains valid until the date of expiry, indicated at the form or until it is withdrawn by the issuing institution.


 

Retirement Pension Application


A person attaining retirement age may be entitled to a Contributory Retirement Pension, if s/he satisfies a set of Statutory Conditions.

You can make use of this form if you have received a notification letter from the Department of Social Security requesting you to provide the details necessary to start off your Retirement Pension assessment. This application form may also be used to apply for a Retirement Pension.

  
 
 

Retrospective Payment of Social Security Contributions Application

 
Eligible applicants can pay back-dated Social Security Contributions which are missing from their respective contribution history, in such a way that they can either qualify for a contributory pension, or else enhance its current or potential payment rate.
 


Senior Citizen Grant Application

 
The Senior Citizen Grant is a onetime payment but paid every consecutive year. Payment is made to those elderly persons who have already reached their 75th birthday or on a pro-rata basis from the date of the 75th birthday up to the end of the current year.
 

 

Social Assistance and Unemployment Assistance Application

 

If you are a Head of Household who may be incapable of working due to medical reasons, unemployed and seeking employment, else registering under Part 1 of the Unemployment Register, or you are following a drug or alcohol rehabilitation therapeutic programme you may apply for Social Assistance or Unemployment Assistance.

Registration for a Social Security Number Application

 
Persons in possession of a permanent Maltese Identity Card will be automatically registered under the Social Security Act (Cap. 318.) once they have reached 16 years of age. Persons not in possession of a permanent Maltese Identity Card between the age of 16 and pension age need to apply for a Social Security Number. This number is only issued once and is valid throughout the lifetime of the applicant.
 
 
 

Students' Allowance Application - Between 16 and 21 years of Age

 
When children turn the age of sixteen and are still attending school and without receiving a stipend, or registering for work but never worked before, are entitled to Student Allowance. Parents can qualify only if their income does not exceed the €24,833.
 
  
 

Supplementary Allowance Application - Head of Household

 
The Supplementary Allowance is intended for persons that have a low income and are not currently in receipt of Children’s Allowance. This allowance is intended to help the eligible persons bridge their income requirements.

Widow's Pension Application


A widow/er, whose spouse satisfies the relevant contribution conditions on his/her death, is entitled to a Widow/er’s Pension or to a Survivor’s Pension, whichever is the most beneficial.
