Medical Certificate Details

A medical certificate must be completed by a medical practitioner and submitted to the Department of Social Security within 10 days (the only exceptions accepted are due to hospitalisation or treatment abroad) from the 1st day of sickness.

It is of utmost IMPORTANCE that the medical certificate has all relevant details (on both sides) adequately filled-in and that it is signed by the applicant before submission.



Note: The front and the back side of the certificate are required to be scanned.

Bank Account Details

Benefit should be deposited in a Savings or Current Account, but not in a Loan Account. The indicated account has to be in the name of the beneficiary. The bank account details provided will also be used for all benefits that you may be currently receiving.
